ELIGIBILITY: The Mead Five Star® Sweepstakes (the "Sweepstakes") is open to all legal residents currently residing in the 50 United States (including D.C.) who have Internet access and are 13 years of age or older at the time of entry. Void where prohibited. Employees (and household family members of such employees) of Sponsor, as defined in Section 9, and its parents, affiliates, subsidiaries and their advertising, promotion and fulfillment agencies, and their affiliates are not eligible. Sweepstakes subject to all federal, state and local laws and regulations.
This Sweepstakes is in no way sponsored, endorsed or administered by, or associated with, Facebook. You understand that you are providing your information to the Sponsor and not to Facebook. The information you provide will only be used in accordance with Sponsor's privacy policy available at www.meadonline.com/privacy.aspx.
2. TO ENTER:
a. Sweepstakes Entry Periods: The Sweepstakes begins at 12:00:01 p.m. ET on 1/9/12 and ends at 11:59:59 a.m. ET on 4/27/12 (the "Promotion Period"). There will be sixteen (16) weekly sweepstakes (each a "Weekly Sweepstakes") and four (4) monthly sweepstakes (each a "Monthly Sweepstakes") held during the Promotion Period. To submit an entry, visit www.Facebook.com/FiveStar (the "Website") and "Like" the Five Star page if you have not already done so. Access the "Five Star Sweepstakes" application and follow the on-screen instructions to provide your full name, street address, city, state, zip code and valid e-mail address to complete your entry submission. You may submit a maximum of one (1) entry during the entire Promotion Period. Your entry will be eligible for the Weekly Sweepstakes corresponding to the Weekly Entry Period in which your entry was made, as well the Monthly Sweepstakes corresponding to the calendar month in which the entry was submitted. General Entry Conditions: Each person may use only one (1) e-mail address to participate in this Sweepstakes, and such e-mail address may be used by only one person regardless of whether the e-mail address is shared by two or more people. Any person who uses more than one (1) e-mail address to enter the Sweepstakes may be disqualified and prizes won may be forfeited subject to Sponsor's sole discretion. Automated entries are prohibited and any use of such automated devices will result in disqualification. All entries become the exclusive property of Sponsor and will not be returned or acknowledged. In the event of a dispute as to the identity of any entrant, the Authorized Account Holder of the e-mail address used to submit the entry will be deemed the entrant. The "Authorized Account Holder" is the natural person assigned an e-mail address by an Internet access provider, online service provider or other organization responsible for assigning e-mail addresses for the domain associated with the submitted address. Each potential winner may be required to show proof of being the Authorized Account Holder. Proof of submitting an entry does not constitute proof of delivery/receipt. Sponsor is not responsible for lost, late, stolen, misdirected, incomplete or illegible entries. By entering, participants agree to be bound by these full Official Rules and the decisions of the judges, which shall be final, binding and conclusive on all matters. All entries must be received by 11:59:59 a.m. ET on 4/27/12 to be entered into the Sweepstakes.
3. PRIZES:
a. Weekly Prizes: Visa Gift Card Winners (20): Five (5) winners will be selected from Weekly Sweepstakes 1, 5, 9 and 13 to each receive one (1) $15 Visa Gift Card and one (1) Five Star® 1-Subject Core Notebook. Best Buy Gift Card Winners (20): Five (5) Weekly Sweepstakes winners will be selected from Weekly Sweepstakes 2, 6, 10 and 14 to each receive one (1) $15 Best Buy Gift Card and one (1) Five Star® 1-Subject Core Notebook. iTunes Gift Card Winners (20): Five (5) Weekly Sweepstakes winners will be selected from Weekly Sweepstakes 3, 7, 11 and 15 to each receive one (1) $15 iTunes Gift Card and one (1) Five Star® 1-Subject Core Notebook. Starbucks Card Winners (20): Five (5) Weekly Sweepstakes winners will be selected from Weekly Sweepstakes 4, 8, 12 and 16 to each receive one (1) $15 Starbucks Card and one (1) Five Star® 1-Subject Core Notebook. Approximate Retail Value ("ARV") of each Weekly Prize: $19. All gift cards are subject to the terms and conditions of the issuer. TOTAL ARV OF ALL WEEKLY PRIZES: $1,520.
b. Monthly Prizes: January Monthly Sweepstakes: One (1) Winner will receive a $75 Visa Gift Card, ARV: $75. February Monthly Sweepstakes: One (1) Winner will receive a $100 Visa Gift Card, ARV: $100. March Monthly Sweepstakes: One (1) Winner will receive a Sony Bloggie Camera, ARV: $149. April Monthly Sweepstakes: One (1) Winner will receive an Xbox Kinect Console, ARV: $299. All gift cards are subject to the terms and conditions of the issuer. TOTAL ARV OF ALL MONTHLY PRIZES: $623.
c. General Prize Conditions: Prizes are nontransferable, non-assignable and may not be redeemed for cash or otherwise substituted except at Sponsor's sole discretion. Sponsor reserves the right to substitute any prize with one of equal or greater value. Sponsor makes no warranty with respect to any prize. The cash equivalent of any portion of any prize is not permitted except at Sponsor's sole discretion. All federal, state and local taxes are the sole responsibility of the winner. The ARV of electronic prizes is subject to price fluctuations in the consumer marketplace based on, among other things, any gap in time between the date the ARV is determined for purposes of these Official Rules and the date the prize is awarded. Any difference between stated ARV and the actual retail price of any prize will not be awarded. TOTAL ARV OF ALL PRIZES: $2,143.
4. PRIZE DRAWINGS / ODDS: Potential Weekly Sweepstakes winners will be randomly selected on or about the Drawing Dates provided in Section 2(a) above, from among all eligible entries received during and prior to the corresponding Weekly Entry Period. Potential Monthly Sweepstakes winners will be randomly selected from among all eligible entries received during and prior to the calendar month for which the drawing is held. Each Monthly Sweepstakes drawing will be held within 3−5 days after the end of each calendar month. For example, the February Monthly Sweepstakes drawing will be held on or about March 5, 2012 and potential winners will be selected from all eligible entries received for the months of January and February. The drawings will be conducted by an independent judging agency whose decisions are final. Fulfillment agent will attempt to notify potential winners by mail at the address given upon entry. If, after two (2) attempts, contact has not been made, prize will be forfeited and an alternate potential winner will be randomly selected, if feasible. Potential winners may be required to complete and return a Winner's Agreement or Address Confirmation and any other documents requested. Sponsor is not responsible for undeliverable, lost or misdirected mail that may result in potential winner not receiving a prize notification. If any prize or prize notification is returned as undeliverable, prize will be forfeited and an alternate potential winner will be selected. Potential winners who receive cumulative prizes valued at $600 or more will be required to sign and return an Affidavit of Eligibility/Release. If winner is an eligible participant but a minor in his/her state of residence, prize may be awarded in the name of winner's parent/legal guardian, who must complete and return any required documentation. Any documentation requested must be returned within the time specified or that potential prizewinner will be disqualified and, if feasible, an alternate potential winner will be randomly selected. Please allow approximately 6−8 weeks for delivery of prize following verification of eligibility. Odds of winning any prize depend on the number of eligible entries received for each drawing. All Prizes will be awarded if the number of eligible entries for each drawing is equal to or greater than the number of prizes available.
5. RELEASE OF LIABILITY/PUBLICITY: By accepting a prize, winner (or parent/legal guardian, if applicable) consents to the use of winner's name, photograph, voice recording and likeness for publicity purposes by Sponsor without additional compensation or permission, where permitted by law. Except where prohibited, all entrants by entering agree that: (a) Sponsor, Facebook, gift card source Best Buy Stores, LP together with its affiliates ("Best Buy") and each of their parents, affiliates, subsidiaries, and the officers, directors, agents, employees of each and all others associated with the development, administration, execution, fulfillment or advertising/promotion of this Sweepstakes are hereby released from any and all liability with respect to, or in any way arising from, this Sweepstakes and/or acceptance or use of any prize, including liability for personal injury, bodily injury, death, damages or loss; (b) under no circumstances will entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, punitive, incidental, consequential, or any other damages, other than for actual out-of-pocket expenses; (c) all causes of action arising out of or connected with this Sweepstakes, or any prize awarded, will be resolved individually, without resort to any form of class action; and (d) any and all claims, judgments, and award will be limited to actual out-of-pocket costs incurred, excluding attorneys' fees and court costs.

GENERAL: Any prize depictions on promotional packaging, point-of-sale, online, or otherwise in connection with this Sweepstakes are for illustrative purposes only and may not represent the actual prizes awarded or received. Sponsor is not responsible for printing or typographical errors in any Sweepstakes-related materials. Sponsor and its affiliates, subsidiaries, parent company and agencies assume no responsibility for any loss or damage to any participant's or to any other person's computer or mobile device relating to, or resulting from, playing or downloading any information or software in connection with the Sweepstakes. Sponsor and its agents are not responsible for technical, hardware, software or telephone/Internet malfunctions of any kind, lost or unavailable network connections, or failed, incorrect, incomplete, inaccurate, garbled or delayed electronic communications caused by the user, or by any of the equipment or programming associated with or utilized in the Sweepstakes, or by any human or other errors that may occur in connection with the Sweepstakes. Any attempts by an individual to access the Web site, mobile site or Sweepstakes technology via a bot script robotic program, script, macro, brute-force attack, or other automated means will result in that individual and their e-mail address(es) becoming ineligible and such entries disqualified. Sponsor further reserves the right to disqualify any individual who tampers with the entry process or attempts to defraud the Sweepstakes in any other way. Sponsor may prohibit a participant from entering the Sweepstakes or accepting a prize if it determines that said participant is attempting to undermine or has undermined the legitimate operation of the Sweepstakes by cheating, hacking, deception or other unfair practices or intending to abuse, threaten or harass other participants. If, for any reason, the Sweepstakes is not capable of being conducted as planned; or if the Sweepstakes becomes corrupted or does not allow the proper execution of the Sweepstakes and/or processing of entries in accordance with these full Official Rules; or, if tampering, unauthorized intervention, actions by entrants, fraud, technical failures, or any other causes, in the sole opinion of Sponsor, corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, Sponsor reserves the right, in its sole discretion, to disqualify any individual implicated in such action and/or to cancel, terminate, modify or suspend the Sweepstakes and conduct a random drawing from all eligible, non-suspect entries received prior to termination to select potential winners. ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE, INTERFERE WITH, OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND/OR CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. Sponsor's failure to enforce any term of these full Official Rules shall not constitute a waiver of that provision.
